V.Outcome Identification

• The client has adequate cerebral tissue perfusion as evidenced by being alert, and oriented, follows verbal commands, and answer questions correctly.
• The client reports reduced pain, facial expression and body movements reflect increased comfort.
• The client demonstrates absence of lung infections or infiltrates.
• The client demonstrates improvement in cognitive functioning and problem solving.
• The client exhibits no signs and symptoms of increased ICP.
• The client does not have accidents and remains injury free.
• The client is afebrile and has no signs or symptoms of local or systemic infection.
• The client does not develop complications from immobility; skin remains intact and no deep vein thrombosis, respiratory problems, or contractures occur.
• The client demonstrates the ability to perform ADLs to the maximum of his functional capacity.
• The client is able to effectively communicate physical and emotional needs to family members, significant others, and health care providers.
• The client’s family verbalize understanding of the client’s condition and demonstrate the willingness and ability to assist him in functioning at his optimum level.
• The client acknowledges his entire body and shows no evidence of unilateral neglect.
• The client and his family demonstrate a positive attitude and coping mechanisms.
